I have an embroidery machine but I don't make tags. I just write on the back of the quilt with a fabric marker.
I put my name, city & state, month & year finished, and if I think about it I may also add the name of the person I made it for but mostly I don't know who's getting it. Oh yea I sometimes will put a name on the quilt if I happen to name it. Sometimes I will and sometimes I don't.

I either write on a separate piece of fabric with permanent pens and then sew the fabric on the back or I print a label out on my printer using "Printed Treasures" paper backed fabric for computer printing. Then I sew it to the back of the quilt.
Some fabric manufacturers have panels of quilt labels that you can buy and write on. And there are quite a few books that you can use a design to trace onto fabric and then sew onto your quilt.
