I have found the same problem with mine.
The rotary blade has to be inserted in the tear-shape opening at the bottom of the slot. You also need to be standing to the left of the slot if you are right-handed or to the right of the slot if you are left-handed. Be sure that your 45 mm blade is sharp enough to cut through all layers.

I absolutely love mine!!! I have the 1/2 inch spaced ruler, the 1/8's ruler, and the one for HST's and QST's. Love them all and plan to purchase the 1/4 inch spaced ruler soon. I had problems with not holding my cutter at the right angle and with waivering with the cutter or moving the ruler, but not since I got these wonderful rulers...very accurate cutting now.

My 6 x 24 rulers are now mostly just used for folding fabric. LOL!

What cutter are you using? I have used an Olfa and now use the Martelli Ergo cutter exclusively. I never have a problem with getting the blade in the slots, and I use the 45mm.
If you are using the cutter that depresses as you cut, that could be your problem. May want to go with an exposed blade cutter.

I love the slotted rulers. I have 2 of the 12 x 12. I have the 18", the 1/4" slots, and the one that cuts in 1//8s. Even have the ones for fringe. I found the one that cuts the Half square triangles the other day hanging behind all my other slotted rulers. Funny thing is, a group of us are getting ready to do a scrappy quilt that uses the 6 1/2" triangles so I was so happy to find a ruler I already had.
