I've seen them used as knobs, also my guild collects them to put a small amount of thread on when teaching kids or adults with disabilities.

If you have grandchildren or young children around you, you could let them paint them and make necklaces out of them. I was in charge of family reunions, and I used to buy the small flags, color the spools the color of the flag, and set several on the tables.

I have many wooden spools of thread displayed on a high shelf in large glass jars in my sewing room, some with thread, some empty. Can't resist if I see them at a garage sale or thrift store.

Someone in my quilt guild actually uses the spools for "fidget quilts" for Alzheimer's patients. Not exactly sure how they are attached, I've never made one.
