Default Pfaff Ambition 1.0 with IDT, puckering

Last year my hubby surprised me on my b'day with a new machine and I have sewn quite a few projects with it, mostly small projects. Didn't notice any problems with sewing anything and have played around with the variety of stitches, buttonholes, etc. I even made a few small quilted items.
Right now I am quilting a bed runner and it is puckering. Those of you with Pfaff machines, probably know the owner's manual is less than optimal. I have the IDT engaged, I lengthened the stitch all the way up to a 4.5 and this puckering does not seem to improve.
Am I missing something?

How did you sandwich the item?

If you pulled the bottom layer to be very tight/taut - when "released" - it will tend to pucker..

I have my pieced top, 100% cotton batting, backing. I pin basted the layers together.
I did note that the top is not puckering, just the backing.

I agree. I pin basted two small quilts last week with flannel as the backing and one doesn't look as smooth on the back. That was the backing I washed.
