This is the same buttonholer that went with the sewing machine I grew up with. Sorry I don't remember how to attach it....when I was about 15, (many eons ago!! LOL), my mom bought a new machine that had a buttonholer feature, so no more attachments. It did make lovely, even, buttonholes. I hope you can figure out how to attach it!!!

Its for making buttonholes. My mom had one like it on her old treddle machine. It makes great buttonholes. The cams are for the sizes. I learned to sew on her old treddle machine. She inherited it from her great grandmother when she was a little girl and still has it. It sews beautifully.

Hi Sally - everyone is correct - it is a button hole maker. The bottom should open and the cams snap into the base. Some where there should be a shank attachment on it (generally right in the front - the bottom of the photo). You unscrewed the presser foot and its attachment and replaced it with this unit. The needle stitched in the sliding unit at the bottom. Those lines are there to help with placement on the garment The top knob helps determine where in the buttonhole starts.
The slider on the left controlled how wide the zigzag was while the cam controlled the back and forward movement. They made great buttonholes. Remember to drop the feed dogs.

That is a great botton holer! My first machine was an old Singer, those metal parts are far superior to the newer plastic accesories they try to pawn off on us. If you're into making your own clothing, you'll get a lot of use out of that. If your machine didn't come witha manual, I would suggest downloading one at the Singer site. Happy sewing!
