Maybe applique a star over it, and a few more stars to make it look planned.

I've had this happen to me twice. The first time I applied some fusible web to a flower and appliqued it, broderie purse style. The second time, used a matching piece with matching thread (again with fusible web), and the patch was practically invisible.

I agree - this is quite doable! If you don't know how to make an invisible ladder stitch, look it up on youtube. You'll be surprised at how easy this will turn out to be, and nobody will ever be able to find it. You might not be able to find it yourself if the little square repeats in the quilt. If it was already quilted, I would say you'd be better off with an applique.

I find it easiest to take out the offending piece and hand stitch a new piece in, however, appliqueing over the damaged square would work too.
