I love this idea. I keep mine at snapfish too and I write down the name of which family member received it. I can see the changes in my quilts as I've progressed and gotten more comfortable sewing.

I have a digital journal on my laptop that includes:
    -design layouts
    -pictures of fabric ideas
    -final draft of quilt
-pictures of blocks on the design wall
-several pictures of finished quilt and the label
    -quilt as gift packaged
-picture of the receiver with the quilt

Each quilt has it's own folder and I title the folder with the date completed.
    ex: 2011-07-1 Baby John Doe
Each file in that folder will be titled "2011-07-1 Baby John Doe 1" - then 2,3,4,etc)

I had been tracking my quilts in a notebook originally. Recently I purchased a software program for tracking my quilts instead. I like it much better and it's easy to import pictures of my quilts. I also think my records are more complete now that I have a standard form to follow for new entries.

I take pictures of most of the quilts i quilt. Once in awhile i might let one get past but for the most part I take a photo. Last year I started keeping a journal of everytime I sewed and what it was that i worked on. They are few and far between entries now as I am in a slump.

My Mom was a quilter and we never kept track of her quilts, and now wish we had. I am keeping track of mine by picture and when made, who got it, and if machine quilted by whom.
Trying to document some of Moms now.

I take a picture of all my quilts, table runners, etc , Note where I got the material, pattern, size, etc with date and if I gave it as a gift, who, and why. i keep the picture in a plastic sleeve in a 3 ring binder in cronoilogical order. Its fun to go back and see my progress - I think.

I keep a quilt scrapbook with pictures and fabric samples, dates, and to whom they were given. Someone told me I should do that back when I started quilting 2 1/2 years ago. I may have missed a few things, but most are in there.

I take pictures of them and write on the back, who they went to, the pattern, and when I made them. I put the pictures in a scrapbook. :)
