    Generally, if the thread issue occurs on the bottom, the top tension needs adjusting (and viceversa.)

    Start with the top tension, tighten by 1/8 to 1/4 turn, and test again.

    Check the wear on the bottom of the foot you are using. Perhaps it is uneven, and contributing to the issue. My other thought is perhaps the needle bar is rotated slightly.
